Privacy Policy

We take your privacy seriously, and we want you to know how we collect, use, share and protect your information.

What information we collect

We respect your right to privacy. All information users enter into the site (registration information, survey responses, links accessed) is kept confidential and used in aggregated, de-identified form to help us understand your needs and to generate summary reports.

We use this information for things like:

  • Generating the individualized feedback provided to you on the results page and longitudinal report
  • Generating reports using aggregated, de-identified data (e.g. pooled summary results for all the individuals within an organization)
  • Operating, evaluating and improving our website
  • We use session cookies to track a visitor's path through our site during a visit, to help us understand how people use our site and interact with us in order for us to continually improve our visitor experience. This process does not track any information after he individual logs out of the site.

You do not need to provide any personally identifying information.

We ask that you create an account by using any email address and password of your choosing. Doing so enables you to log-in and track your scores over time. You do not need to provide your name during registration. If you wish to print or e-mail a Certification of Completion that documents ONLY the dates you have taken the Well-Being Index you will have the option to enter your name. Doing so is not required, and your name will not be stored or linked to your responses. In order to have a password recovery function we do ask for an e-mail address. By providing this information the system can automatically send you an e-mail, at your request, if you forget your login/password information. You can also choose to have future communications from us (e.g., reminders to complete the index in 6 months) sent to this e-mail address.

During the registration process we also ask that you provide limited demographic information. This information allows the program to tailor your individualized results page by gender. We also ask your gender, age, and occupation.This allows the system to generate reports using aggregated, de-identified data from all individuals at an organization which are provided to a designated organizational administrator and used to provide feedback on how individuals are doing. No individual data is forwarded.

We do not save personal information to use for other purposes, nor do we provide it to any other organizations.

You may decide at some point that you no longer wish to receive communications from our site. To stop receiving communications, you can unsubscribe your email address.

IP addresses

The Web server automatically collects the Internet Protocol (IP) address of the computers that access our site. An IP address is a number that is assigned to your computer when you access the Internet. It is not truly personally identifiable information because many different individuals can access the Internet via the same computer. We use this information in aggregate form to understand how our site is being used and how we can better serve visitors. We will not use share your IP address.

Protecting your privacy

We use reasonable security measures to protect the confidentiality of personal information under our control and appropriately limit access to it. We use a variety of information security measures to protect your online transactions with us. The site uses encryption technology, such as Secure Sockets Layer (SSL), to protect your personal information during data transport. SSL protects information you submit via our website. That being said, we cannot ensure or warrant the security of any information you transmit to us, and you do so at your own risk. We have taken reasonable steps to ensure the integrity and confidentiality of personally identifiable information that you may provide. You should understand, however, that electronic transmissions via the Internet are not necessarily secure from interception, and so we cannot absolutely guarantee the security or confidentiality of such transmissions.

Privacy policy updates

We may need to update our privacy policy as technology changes and evolves. If we make significant changes to the privacy policy, we'll post a prominent message on our websites.
